Health Insurance: Per company policy Reports to: Tannia Lascano, Community Health Director Supervises: None Job Purpose: The Bilingual Community Outreach Community Health Worker acts as an advocate and champion for LifeSpring’s patients and their families and the community. Working directly with them, this role promotes physical, mental, emotional, economic and spiritual health for families in urban Chattanooga by reducing barriers to health care, promoting healthy behaviors and connecting families to supporting resources. The CHW’s role is central to LifeSpring’s model in facilitating integral health and supporting the work of LifeSpring Medical Staff. LifeSpring’s CHWs are highly relational; they love to engage with and encourage patients, while building trust and gently holding them accountable to improved health practices.
Fluent in Spanish

High school graduate or equivalent required, post-high school certification or degree preferred
Prior experience in community health or social services preferred
Adheres to and engages in the Mission of LifeSpring Community Health: LifeSpring exists to demonstrate the love of Jesus Christ by providing a pediatric clinic with exemplary medical staff and innovative services that embrace the whole child, serve their family, and transform the health of our community. Work with patients and their families to set goals for a healthier lifestyle and support them as they attain a positive change. Follow-up with patients via phone calls, home visits and visits to other community settings where patients can be found. Stay up to date on services provided by local government, nonprofits, churches and other organizations and connect families to supporting community resources as necessary.
Assist LifeSpring in building relationships with the schools and afterschool programs where patients attend. Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families for healthy behavior intervention focused on but not limited to healthy eating and physical activity. Present feedback to LifeSpring staff on community concerns, needs and desires.
Represent LifeSpring at community events including some weekends and evenings. Implement community outreach programs in which health outcomes can be measured, evaluated and reported.
Help identify health equity champions in their community and record their success stories.
Meet routinely with the Community Health Team, attend staff training and participate via technology or in person regarding projects with partner organizations.
